In order for this appeal to be submitted to the Appeal Committee, you must have a completed file in the Financial Aid Office. Extenuating circumstances must exist and be addressed for all semesters in which you failed to meet Satisfactory Academic Progress standards. Items in red are required.

Remember to submit a completed curriculum guide for the degree or certificate in which you are appealing, as well as any supporting documentation for your appeal to the Financial Aid Office in C-210 by the appeal deadline. No late appeals or additional documentation will be accepted after the deadline.

You will be notified in writing of the committee’s decision and recommendations.
